Xbox keeps freezing when I change to controller

so when ever I try and switch to controller my controller my game just freezes and I can't do any thing with out reseting my whole xbox.

How are you switching? You simply pick up the controller to play with it. You can actually play with a hand on controller and a hand on mouse if you wanted, without any switching.
