Controller wont connect Via KM2, Blinking Controller Light

Controller will not connect, 360 displays "reconnect controller"

Connections appear correct, Powered via a wall outlet and other usb mini into back of XBOX.

Keyboard, mouse, and gamepad are all in correct slots.

Using wired xbox 360 microsoft controller (default)

Keyboard: Razer Black Widow

Mouse: G Wolves Skoll
